What Is Sleep Apnea?

It's more serious than you think

This disorder has been labeled the silent killer. Sleep apnea is defined by continuous involuntary breathing interruptions during sleep. During sleep, some people experience interrupted sleep caused by gasping, loud snoring, and momentarily stopping breathing. The sleep disruption is due to not getting enough oxygen while sleeping, which causes other mental and physical issues from lack of sleep. Each episode can last just a second or as long as a couple of minutes. Patients with severe sleep apnea may experience as many as 30 breathing interruptions within an hour. There are a variety of issues that may contribute to your sleep apnea. Non-medical factors may include stress, lack of proper nutrition, poor sleeping habits, and lifestyle factors. Medically, sleep apnea can be caused by a deviated septum or polyps in the sinuses, making it difficult to breathe while sleeping, Parkinson's disease, diabetes, high blood pressure, anxiety, or clinical depression.

Most importantly and in the majority of cases, sleep apnea is caused by the position of the tongue causing an obstruction of the airway. A correct diagnosis is the first step in resolving your condition. If the sleep apnea is in a milder phase, your doctor will suggest changes to your lifestyle, like quitting smoking, losing weight, avoiding tranquilizers and sleeping pills, and regular exercise. Your nasal cavity must be examined to eliminate any contributing allergy or other blockages. If these conventional methods are not helping, there are other methods for relieving this medical problem.

Types Of Sleep Apnea

What Causes Sleep Apnea?

Obstructive vs. Central Sleep Apnea

For our patients in Beverly Hills & Sherman Oaks sleep apnea causes are many and varied, and identifying them can help you treat or prevent the condition and ensure that your sleep is optimally restful. Depending on the cause, sleep apnea is classified as “obstructive” or “central.” Obstructive sleep apnea describes episodes caused by decreased airflow through the nose or mouth and is the most common form of sleep apnea. Central sleep apnea results from a problem with the brain’s ability to maintain constant breathing. Some patients experience a combination of OSA and CSA, commonly referred to as complex sleep apnea.

Airway Obstruction Due to Inadequate Tongue Space

This is the most common cause of snoring and sleep apnea and is not diagnosed by most medical professionals. If your mouth does not have enough space to accommodate your tongue, your tongue may obstruct your airway while you sleep. This condition is often caused by misaligned jaws, TMJ problems, genetics, and inadequate orthodontic treatments.

Excess Weight

Excess weight is a prevalent and contributing factor to obstructive sleep apnea. More than half of patients with OSA are overweight or obese. Excess fat surrounding the upper airways reduces the volume of the airway and increases the risk of OSA.

Excessive Overbite

Overbite refers to the way the upper teeth cover the lower teeth. An excessive overbite causes the lower jaw to sit farther back, resulting in the tongue obstructing the airway and causing OSA.

Enlarged Tonsils & Adenoids

Enlarged tonsils and adenoids may result from allergies or other causes and are the most common causes of OSA in children. Tonsils and adenoids comprise lymphatic tissues, with the tonsils positioned at the back of the throat and adenoids behind the nose. When these tissues become inflamed, swelling may impair airflow through the mouth or nose, leading to OSA.

Tongue Tie

Tongue tie is a condition where the tongue is attached to the front part of the floor of the mouth, preventing the tongue from resting against the palate. This problem needs to be corrected in children as early as possible to prevent future complications.

Deviated Septum

A deviated nasal septum can impair airflow through the nose and contribute to OSA. Patients with deviated septums may also experience increased congestion or nosebleeds.

Alcohol & Other Substances

Alcohol, sedatives, tranquilizers, and other substances can also cause sleep apnea by causing the muscles in the throat to become overly relaxed.

Smoking

Because smoking inflames the upper airways, it can also increase the chance of OSA. Furthermore, smoking has been shown to delay the brain’s response to breathing interruptions, potentially leading to longer, more dangerous episodes.

Genetic Factors & Medical Conditions

Genetic factors like a naturally narrow airway, a larger neck circumference, or family history of sleep apnea also put some patients at a higher risk. Medical conditions like asthma, hypertension, Parkinson’s, and hormonal disorders can also play a part in the development of sleep apnea.

Menopause

Menopause causes progressively declining levels of hormones estrogen and progesterone, both of which aid with sleep. Hormonal changes during menopause can also contribute to sleep apnea by affecting the brain's breathing mechanisms.

Dangers Of Snoring And Sleep Apnea

  • Depression
  • Diabetes
  • Fatigue
  • Heart Attack
  • High Blood Pressure
  • Impotence
  • Intellectual Decline
  • Memory Loss
  • Stroke
  • TMJ And Sleep Apnea

TMJ involves a misalignment of the jaw joints. This can alter the resting position of the tongue, which can block the airways as you sleep. This can potentially lead to obstructive sleep apnea, as the lower jaw is directly attached to the tongue. With TMJ, the tongue adapts to sit farther back than its natural position, acting as a pillow to cushion the jaw and relieve muscle tension. This continues into sleep, preventing air from getting into the lungs, much like when the back of the throat softens up and collapses in non-TMJ-related sleep apnea.

Sleep Apnea Treatments

Discover treatments that can help relieve snoring and sleep apnea

Dr. Eddie Siman provides his patients with painless, non-surgical, proprietary holistic therapies. Unlike other medical professionals who treat TMJ, such as orthodontists, ENTs, and neurologists, Dr. Siman's approach to treating sleep apnea puts him in a class all his own. He utilizes advanced technology to get to the root of your problem and help alleviate your sleep apnea.

Traditional Methods

CPAP

Continuous positive airway pressure (CPAP) is a machine that delivers high-pressure air through a mouthpiece that covers your nose and mouth. The more considerable air pressure assures constant opening of the upper part of your lungs. The patient wears the mask throughout the night. This method effectively prevents and eliminates apnea and snoring for patients suffering from mild to moderate sleep apnea.

Additional Reading

However, more severe sleep apnea symptoms may require patients to have more invasive procedures to find relief. However, the major obstacle of using the CPAP is the lack of proper fitting, constantly adjusting the mask, and portability or indiscretion of wearing the CPAP. In addition, this breathing device is costly, and the costs of replacing parts every three months like tubing, filter, and chamber replacement can add up — not to mention the constant need to clean the machine properly. Most recently, numerous manufacturers of CPAP devices have recalled their machines because these devices were causing respiratory infections due to bacterial accumulation in the masks and hoses that patients could not sterilize adequately. This is a serious factor for patients with respiratory problems and may be life-threatening, especially in the elderly population.

What Is The Alternative?

Oral Devices

Many patients may also find relief using oral devices to eliminate sleep apnea. These hygienic and comfortable devices keep the airway open by bringing the jaw forward and holistically preventing the tongue from blocking the throat. These devices are very easy to use, and Dr. Siman uses his advanced equipment for the optimum fabrication of a device, making sure that the muscles of your face, neck, and jaw are in a relaxed position while you wear the device. In Sherman Oaks, we offer the following sleep apnea solutions:

Consultation and Evaluation

Dr. Siman's clinical examination includes a diagnosis of your sleep apnea-related symptoms, including migraines, headaches, ringing in the ears, neck and shoulder pains, snoring, sleep apnea, and more. Dr. Siman will evaluate your airway through the analysis of your CT scan and schedule you for a sleep study that will be done in the comfort of your own bed through the most advanced technologies available. The obtained data will be reviewed by a board-certified sleep specialist that will determine if you are a candidate for an oral device. In diagnosing sleep apnea, you as a patient play a significant role in presenting all of the symptoms — when they began (even those that seem unrelated to the problem), critical personal information, any sleeping disorders, any medicines and supplements, and recreational drug use.

In-house CT Scan

Our state-of-the-art in-house CT Scan will help Dr. Siman determine how your Sleep Apnea-related issues impact your body.

Custom-fit Orthotic Device

Dr. Siman wants to give his patients the best chance to overcome their sleep apnea, even if it means creating the device himself. So he custom designs his sleep apnea devices based on FDA-approved biocompatible materials to eliminate snoring and increase the longevity of his patients' lives. He has been able to help many eligible patients eliminate their CPAP machines, return to the bedroom, and in turn, improve the overall health of his patients. Dr. Siman's device is comfortable, adjustable, and tolerable to wear for long periods compared to traditional CPAP machines. It also requires no electricity or batteries. The device fits like a retainer with the help of dental pictures and muscle testing for optimized placement. Experience a restful sleep with no noise, no discomfort, and less infection risk.

Neuromuscular Therapy

Dr. Siman uses physio-therapeutic equipment, such as TENS, to relax tense jaw muscles. The transcutaneous electrical nerve stimulator (TENS) delivers pulses of electricity to your body's neurotransmitters, so the neurons release endorphins and signal your muscles to relax. Tongue movement can be controlled with a nerve stimulator implanted under the skin on the right side of the chest. The hypoglossal nerve is stimulated and prevents the tongue from blocking the throat.

Pediatric Sleep Apnea

Sleep Apnea Affects Children Differently Than Adults

The condition of sleep apnea is caused by the narrowing or blockage of the upper airway while sleeping. However, sleep apnea affects children and adults differently. For example, most adults experience brain fog, fatigue during the day, and obesity, whereas children tend to act out with more pronounced behavioral problems. What Is Pediatric Sleep Apnea?

Studies show that a large percentage of children suffer from obstructive sleep apnea. It can develop at any age. Two types of sleep apnea can occur in children — obstructive sleep apnea and central sleep apnea. Central sleep apnea occurs when the part of the brain responsible for breathing is not functioning correctly. The most crucial difference between the two types of apnea is the frequency of snoring. It is more frequent in obstructive sleep apnea because it is related to airflow obstruction. Pediatric sleep apnea or obstructive sleep apnea (OSA) is a condition when the child's breathing is wholly or partially blocked during sleeping and happens repeatedly.

The condition is relatively common and also, as a disease, results in a considerable range of health complications. The consequences of untreated obstructive sleep apnea can lead to various health and behavioral problems. Any blockage in the mouth cavity and throat will partially or entirely block the airflow while sleeping and make the child snore or stop breathing. These breathing interruptions can happen more than 30 times in one hour and last for 10 to 20 seconds. Quality of sleep is extremely low because the brain awakens when the breathing stops — resulting in an unrestful sleep.

Pediatric Sleep Apnea Symptoms

The most common cause is enlarged tonsils and adenoids, neuromuscular disease, or craniofacial abnormalities in children. In addition, infection or inflammation can cause these glands to swell. When the glands are enlarged, they block the airway flow during sleeping. Obstructive sleep apnea can also be caused by being overweight, a tumor or growth in the airway, congenital disabilities, or specific syndromes like Down syndrome or Pierre-Robin syndrome. Obesity or increased body mass index is also a common factor for pediatric sleep apnea, leading to cognitive vulnerability.

Many children are misdiagnosed with A.D.D. and A.D.H.D. due to their untreated obstructive sleep apnea. These children are deprived of adequate amounts of oxygen while they sleep, which causes them to feel tired throughout the day, in the classroom, and during their interactions with others. If your child has been diagnosed with A.D.D. or A.D.H.D., it is highly recommended not to rule out the possibility of OSA. Symptoms may be different in each child but overall include:

  • Snoring (loud snoring or noisy breathing during sleep)
  • Periods of no breathing or noticing there is no movement of the chest
  • Restless sleeping — including moving while sleeping, sleep terrors, bed-wetting or nocturnal enuresis, nighttime sweating, gasping for air, and sleeping in odd positions like arching their neck backward to open the airway.

Also, a behavior difference is noticed in your child during the day. For example, children who suffer from pediatric sleep apnea often have sleepiness, irritability, hyperactivity, learning problems, and morning headaches.

Finding The Perfect Fit For Your Child

Dr. Siman designed and prototyped his sleep apnea device based on FDA-approved materials and appliances to improve his patients' sleep quality. The device is comfortable, adjustable, and tolerable to wear for long periods compared to traditional CPAP machines. It also requires no electricity or batteries. The device fits like a retainer, making it easier for kids to tolerate while sleeping. In addition, there is no noise, no discomfort, and less infection risk compared to traditional CPAP machines.

Pediatric Sleep Apnea Solutions

A detailed physical examination is needed to evaluate your child. The assessment includes observing your child's general appearance (paying careful attention to cranial characteristics) and evaluating the upper airway by visualization and X-rays. A lateral neck X-ray can be performed in the office on the same day of your appointment to rule out an enlarged adenoid. It is also necessary to eliminate nasal obstruction depending on your child's age. Septal deviation may be considered in infants and nasal polyps in older kids. Dr. Siman may suggest a sleep study test or polysomnography. It is a test that diagnoses sleep disorders while recording brain waves, oxygen levels in the blood, heart rate, breathing, eye, and leg movements during testing. In Beverly Hills & Sherman Oaks, we offer the following sleep apnea solutions:

  • Jaw Alignment
  • In-house CT Scan
  • Neuromuscular Therapy
  • Custom-fit orthotic device

While performing various tests, children require a friendly atmosphere and approach, along with smaller and specialized equipment. If left untreated, obstructive sleep apnea and obesity can lead to a significant decrease in your child's quality of life. For example, this condition leads to decreased interest in daily activities and relationships with families, friends, and school. In addition, Pediatric Sleep Apnea can cause your child to have less oxygen in the blood than average, leading to permanent damage to the heart and lungs and poor growth and development.
